Access to Primary Health Care

Access to Primary Health Care

Access to Primary Health Care (APHC) is a program based on a social model of health that aims to improve access to primary health and wellbeing services for Indigenous people living in the Rockingham and Kwinana communities.

The focus of the program is on promoting healthy families and communities through preventative services such as

  • Promoting and supporting access to health services;
  • Facilitating health screening for chronic diseases like Asthma, diabetes, and Heart Disease;
  • Child health and developmental screening;
  • Immunisation;
  • Parenting support
  • Women’s health checks;
  • Health promotion and information; and
  • Health information sessions and presentations to groups.

The program is able to assist with transport for home health visits and for primary health care clinic appointments.

Aboriginal Health Workers are also available to accompany clients to appointments if clients are reluctant to go on their own and would benefit from the extra support.

Since the programs inception in 2006 it has expanded to include the following services.

Antenatal Clinic

APHC Antnatal Clinic

We provide information and antenatal care to support Indigenous women through pregnancy to help them deliver a healthy baby. The clinic runs from Rockingham one week and Kwinana the following week.

Our clinic operates as a shared care clinic with a local Midwife and an Aboriginal Health Workers and an Aboriginal Receptionist to welcome clients. No appointment is necessary as the clinic operates on an open appointment basis.

We are also able to offer antenatal care in the home through home assessments if required.

How to access this service
The service is open to Aboriginal and Torres Strait Islander women living in the South Coastal region. No appointment is needed. The service is free of charge.

Post Natal Care

We provide a post natal home visiting service and clinic with a Midwife and Aboriginal Health Worker. The program follows through with support from the early discharge program with maternal and child 6 week checks. It also links into the Rockingham Kwinana Community Health Services Child Health Nurses.

By keeping in contact with families we are also able to provide support with

  • Immunisation;
  • Developmental milestones checks;
  • General health checks;
  • Linking up with child health clinics and playgroups; and
  • School linkages.

Men’s Health Screening

The program offers a range of supports and services for men with an aim to improve long term health outcomes for Indigenous men living in the region.

We assist male clients to arrange health checks for chronic diseases or prior to job interviews. Through collaborative arrangements with 3 different local medical centres we aim to ensure male clients receive double appointments within the week of request.

We can also assist with

  • Health and illness information; and
  • Information for dads

Play Groups

Creche Facilities

Working in collaboration with other local services we are able to offer two facilitated playgroups for Aboriginal parents and their children.

Town of Kwinana

Operates once a week at Nanakaat, Hutchens Cove, Darius Building and is supported by an Aboriginal facilitator and a Ngala Aboriginal Early Childhood facilitator.

Women’s group

YOK KADADJINY operates on Tuesdays in school terms between 10am and 12md at Medina Aboriginal Cultural Centre, 13 Leasham Way, Medina.

Aboriginal Community Centre Babbingur Mia

Access to Primary Health Care

The centre is located at 61 Council Avenue, Rockingham. Access is via the rear of the Aquatic Centre car park. The centre offers a range of activities including:

  • Women’s yarning and craft group
  • Health Checks through the Rockingham Kwinana Division of General Practice.
  • Information and health checks offered by male Aboriginal Health Workers are available 2 mornings per week.
  • Public access to computers word processing.
  • A venue for cultural celebrations and events.
